Coopertown Middle School: Home of the Eagles!

REGISTRATION INFORMATION

The following items are required for school enrollment:

 

1. Birth Certificate

2. Social Security Number

3. Documentation of Immunizations on certified TN form:

 

 

At a minimum, the following items should documented:

  • 4 doses of Diptheria/Pertussis/Tetanus (DPT)
  • 4 doses of Polio
  • 1 dose of Measles/Mumps/Rubella (MMR) after the first birthday

4. A current physical for initial enrollment in Robertson County

5. Proof of legal residence:

 

A minimum of three(3) of the following:

  • mortgage documents/property deed
  • apartment/home lease
  • utility bill
  • driver's license
  • voter registration card
  • automobile registration
  • notarized statement of residency
  • other verifiable items bearing the name and address of parents/guardians

6. If applicable, a certified copy of any custodial decree or petition establishing the district resident as the legal guardian of the student and further declaring that the guardianship was formed for a purpose other than to establish residency for school attendance purposes.

 

Once enrollment is approved, the following items will be completed:

 

1. Data form (includes student information, parent/guardian information, emergency contact information, other children in the home, school history, and medical information)

2. Proof of grade placement (transfer slip or report card from previous school).  If previous placement was in a non-approved school, testing must be conducted to determine grade placement.  This may include students coming from a home-school setting.

3. Release of student records (from previous placement)

4. Home Language Survey

5. Migrant Education Program Occupational Survey

6. Medical History Form

7. Acknowledgement Form of Robertson County policies and procedures
